Office Policies
Valid Identification
A valid photo ID and all current insurance cards must be provided at your appointment. Patients will be responsible for charges incurred if current insurance information is not provided at your appointment.
Dr. Joyce Lewis or Dr. Dorothy White-Williams must be assigned as your PCP when permitted prior to your appointment.
Appointments
Cancellation Policy
All appointment cancellations must made within 48 hours of your scheduled appointment time. Cancellations made less than 48 hours will incur a $50 late cancellation fee.
No Show Policy
No shows will be charged a $50 fee. After three no shows, you may be dismissed from the practice.
Late Arrivals
We encourate on time arrival to all appointments. If more than 15 minutes late for an appointment, you may be asked to reschedule.
Payments/Charges
Copays are due at the time of your appointment. Past Due balances are due at the time of your appointment, or appointment may be rescheduled.
Patients are responsible for all remaining balances after insurance companies are billed. An active health insurance policy does not guarantee that all medical claims will be covered. Not every charge or type of service is covered under a plan. Actual benefits are subject to all plan terms, definitions, limitations, and exclusions in effect on the date of service. Patients are responsible for balances unpaid by their insurance company.
Self-pay prices are due at the time of service, or the appointment will be rescheduled.
Paperwork/Form Fees
Scheduled visits with forms will incur a $40 fee. Pop-up forms will incur a $50 fee.
Form Fees Excludes: Health Physical Forms/School Physical Forms/Immunization Forms for school.
Note: Excluded forms must be presented at the time of appointment or no later than 10 days after. Patients may be charged $15 for forms presented after 10 days.
